As far as I know from playing dr_horrors, the first trap is designed intentionally that way so skilled players can jump out before they get killed.

I'll leave this open for someone who plays more deathrun like @Buster or @Alex-Ario to answer.

You don't have a demo, so it's not clear how he got up there. He could have gotten a rtd that could have helped him, he could have bhopped, or maybe he was just able to parkour his way up there.
