Problem with a Merc die twice roll

So I have this quiet for a while. Whenever I roll Mercs die twice. I somehow legit died. Has anyone else also having this issue?
What I meant is if you get killed. You somehow still died and can’t revive yourself. I had it plenty of times at 25 seconds and just removed it after I died

How did you die? as far as I know, the RTD makes you invisible and invincible if you "die", so if you actually die in a place where you can't be respawned, the RTD is void.
